Full Job Description
o To accept health care referrals, assess need and oversee all health care assessment to support service user's access timely treatment.o Provide harm reduction advice, information and support with an emphasis on safer drug use, safer sex, HIV, Hepatitis A, B and C and other blood borne infections o Be responsible for maintaining a robust health care case load supporting the health inequalities agenda.o To carryout clinical interventions such as urine screening and CIWA-Ar and COWS, vital sign assessment, ECG's including blood pressure, and other physical health checks.o To carryout alcohol and drug detoxes supported by the Band 7 Nurse Prescriber.o To carry out comprehensive assessments, construct care packages with multi-disciplinary colleagues where required, review care plans in a structured format and manage onward referral., Please see the attached job description for the full list of duties and responsibilities. To provide a range of structured psycho-social interventions including CBT based brief solution focused therapy and motivational enhancement techniques. To work with a range of professional in ensuring service users needs in terms of safeguarding and mental health are met.
