Job overview Band 4 - Clinical Shift Allocator An exciting opportunity has arisen to join the Rehabilitation and Frailty Team (RAFT). We are looking for an experienced and motivated health care worker who is passionate about providing high standard of patient centred care. You will be working closely with community nurses, community matrons/ care coordinators, physiotherapists, occupational therapists, and rehabilitation assistants who are committed to maintaining independence, promoting self-care and assisting patients to achieve their goals. The successful candidate will be hardworking, reliable and able to use their initiative and work flexibly to respond to the service needs. You will be part of a friendly and approachable team, and will be well supported by your colleagues and line manager., Main duties of the job The Rehabilitation and Frailty Team currently operates from 08:00 to 20:00 every day, and core hours are between 09:00 and 17:00 Monday to Friday. Weekends and Bank Holidays are covered on a rota. We cover Basildon, Wickford, Billericay, Brentwood and Thurrock areas. The post holder will work as a Clinical Shift Allocator within Rehabilitation and Frailty Team as part of a multidisciplinary team providing care and see a wide variety of patients predominately with physical conditions at home. The post holder will be responsible for a delegated caseload as directed by qualified staff.
