Full Job Description

We are currently seeking a highly skilled and motivated individual to support the team to manage a complex caseload of patients and provide leadership for the administrative element of the team. Alongside these duties, as part of the senior team, you will be expected to support the junior members of the multidisciplinary team and support the clinical services manager in day-to-day management and service development. The postholder will ensure that all service procedures and processes are effective and safe and enable the service to meet its contract targets and deliver a high-quality patient experience.   • Registered Nurse- NMC Nursing Certificate, DiP, Degree
  • Evidence of professional revalidation
  • Experience of working with/within a multi-disciplinary team
  • Experience of supervision and line management
  • Completion of advanced skills assessment course, Clinical Decision-Making course , Sepsis course, Deteriorating Patient course
  • Independent non-medical prescribing (V300)
  • Knowledge of safeguarding adults/children and completion of mandatory training
  • Practice Supervisor & Assessor
  • Palliative care qualification/proven experience / management of the dying patient
  • Specialist Practice District Nursing or Rapid Response experience
