Clinical Practitioner - Front Door

The Mental Health Assessment Practitioner (Band 6) plays a key role within the City and Hackney Front Door Service, including the Crisis Pathway, delivering a responsive, safe, and high-quality mental health assessment service. The post holder works as part of a multidisciplinary team to assess individuals presenting with both urgent and non-urgent mental health needs across a range of settings, including emergency departments, acute hospital wards, and the crisis hub.

The role involves conducting comprehensive, face-to-face biopsychosocial and risk assessments, developing person-centred care plans, and supporting individuals in crisis through effective intervention and signposting. Practitioners promote recovery-focused practice and work collaboratively with service users, carers, and partner agencies to ensure continuity of care and positive outcomes.

The post holder is expected to work flexibly across the service, including shift work within a 24/7 rota, and may support different teams such as the Psychiatric Liaison Team or Home Treatment Team. Additionally, the role includes contributing to staff education, maintaining accurate clinical records, and upholding safeguarding and professional standards. This position requires strong clinical judgement, excellent communication skills, and the ability to work both autonomously and within a multidisciplinary environment., The main duties of the Mental Health Assessment Practitioner centre on delivering high-quality assessments and interventions for individuals experiencing mental health difficulties or crisis. The post holder is responsible for undertaking timely, comprehensive assessments, including detailed risk evaluations, and developing collaborative care and crisis safety plans tailored to individual needs.

The practitioner will respond to emergency and routine referrals within agreed timeframes, coordinating care and liaising effectively with referrers, multidisciplinary teams, and external agencies. They will implement and evaluate care using evidence-based psychosocial interventions, supporting service users to manage their mental health and promoting recovery and independence.

Key responsibilities include maintaining accurate and up-to-date clinical records, providing clear communication and reports, and ensuring adherence to safeguarding and professional standards. The role also involves supporting carers, facilitating referrals to appropriate services, and addressing service users' physical health needs where identified.

Additionally, the practitioner contributes to team functioning through shift coordination when required, supervision of junior staff or students, and participation in quality improvement initiatives. The post holder must manage their workload effectively while contributing to a safe, responsive, and person-centred service.
