Full Job Description
As demand within Primary Care continues to grow, we recognise the vital contribution Clinical Pharmacists make in improving patient outcomes, supporting safe prescribing and helping patients get the most from their medicines. Therefore we are looking to recruit an enthusiastic and motivated Clinical Pharmacist Working alongside our Lead Pharmacist, GPs and wider clinical team, you will play a key role in delivering high-quality medicines optimisation, supporting safe prescribing and improving patient outcomes. This role offers an exciting opportunity to become part of a well-established clinical team where your expertise will be valued, your ideas welcomed and your professional development actively supported., As an experienced Clinical Pharmacist, you will become an integral member of our multidisciplinary team. Working closely with GPs and the Lead Pharmacist, you will provide expert pharmaceutical care that improves patient outcomes whilst reducing medicines-related risk. This role combines direct patient care with medicines optimisation, prescribing support and service development. You'll be encouraged to work autonomously whilst knowing that clinical support is always available.The successful applicant will be involved in: The successful applicant will be involved in:
- Structured Medication Reviews (SMRs)
- Medicines optimisation and medicines management
- Managing repeat prescribing systems
- Medicines reconciliation following hospital discharge
- Reviewing Docman correspondence and implementing medication changes
- Responding to medication queries from patients and clinicians
- Supporting safe, effective and cost-efficient prescribing
- Working collaboratively with GPs, nurses and the wider multidisciplinary team
- Clinical audit and quality improvement initiatives
