Full Job Description
City Health Care Partnership CIC is recruiting a Clinical Pharmacist to support the Frailty Service in Kingston upon Hull. You will work with doctors, nurses, technicians and therapists to perform comprehensive geriatric assessments across various settings.
The role involves travel across the service area, regular medication reviews, and collaboration with a busy Medicines Management team. Flexible hours and secondment arrangements may be considered.
