Clinical Pharmacist - Elective Orthopaedic Centre

The centre specialises in elective bone and joint surgery and sees patients from across 12 hospitals and 8 boroughs. It is a fast-paced, dynamic environment, with continual improvements being made to discharge pathways. The post-holder will be responsible for providing a safe and effective clinical pharmacy service to meet the needs of the enhanced recovery pathway. The post-holder will support in the management of the ward Pharmacy service, including suggesting, implementing and reviewing service improvement changes. Additionally, the post-holder will be involved in providing a Pharmacy pre-op service to elective surgical patients across London North West University Healthcare NHS Trust. Main duties of the job The post holder is responsible for:

• Providing a safe and effective clinical pharmacy service from admission to discharge on the Elective Orthopaedic Centre in accordance with Trust policies.
• Providing accurate medication reconciliation, in order to minimise medication related errors on admission.
• Maintaining a high level of pharmaceutical input, as part of a multidisciplinary team within the department and on the wards, to ensure optimal medicines usage and value for money.
• Facilitating a speedy discharge for patients by organising their medication according to Dispensing for Discharge policies.
• Liaising with the multidisciplinary team to facilitate the successful implementation of the same day discharge pathway
• Liaising with the multidisciplinary team and be ward based, to improve communication with patients, hospital doctors, nursing team and GPs., The post holder provides a pharmaceutical service to the Trust on a rota basis at weekends, late duties, bank holidays, Christmas and on-call.
  
  Master's Degree in Pharmacy (MPharm)
• Certificate in Pharmacy Practice or equivalent experience
• Member of the General Pharmaceutical Council
Desirable
• Post graduate diploma in pharmacy practice
• Independent prescriber, Good, broad clinical knowledge and ability to apply to a ward and dispensary setting
• Relevant post-graduate experience
• Some experience of providing training, mentorship and supervising staff
• Documented evidence of completed project work, audit and practice research
• Evidence of Continuous Professional Development
• Apply knowledge to practice
• Willing to take responsibility
• IT literacy including word processing, e-mail and use of the internet
