Clinical Pharmacist

We are seeking an experienced and motivated Clinical Pharmacist Independent Prescriber to join our team. This is a patient-facing role suited to a confident clinician who is comfortable working autonomously and managing a wide range of presentations within primary care., Independently assess, diagnose, and manage patients, including those presenting with undifferentiated conditions

- Prescribe safely and effectively within scope of practice

- Manage acute and chronic conditions in line with current clinical guidelines

- Work autonomously while contributing to the wider multidisciplinary team

- Support and deliver high-quality patient care within a busy practice environment

- Demonstrate a strong understanding of QOF (Quality and Outcomes Framework) and QIF (Quality Improvement Framework)

- Contribute to achieving practice targets and supporting day-to-day operational and clinical matters

- Provide clinical support and advice to colleagues where appropriate

Qualified Clinical Pharmacist with Independent Prescriber status

- Proven experience in primary care or general practice setting

- Strong clinical assessment and decision-making skills

- Ability to confidently manage undifferentiated presentations

- Excellent communication and interpersonal skills

- Proactive, organised, and able to work independently
