Clinical Nurse / AHP Specialist

An opportunity has arisen for a part time Band 6 Glaucoma Nurse Specialist within St.Paul's Ophthalmic Outpatients. This post is part of a Multi-Disciplinary Team dedicated to the assessment , monitoring and treatment of patients with Glaucoma . It will involve seeing Glaucoma patients within the outpatient department in nurse led clinics. The post holder will act as a point of contact for existing and newly diagnosed glaucoma patients. The post holder will contribute to Research , Audit and Practice Development. Will deputise for the Band 7 manager. It is essential that the applicant has at least 2 years experience working within an Ophthalmic setting. The applicant must possess a relevant Ophthalmic Nurse Specialist qualification. An ANEX 21 ( training post ) may be considered in order to gain the required competencies.( 75% of the Band 6 will be paid until completion of competencies ) PREVIOUS APPLICANTS NEED NOT APPLY Main duties of the job Perform specialist assessment of patients nursing needs, plan, implement andevaluate care delivery using advanced clinical knowledge and clinical skills.? Lead clinical care by managing a patient caseload providing expert assessment,planning and evaluation.? Co-ordinate and manage nurse/AHP-led clinics (within Speciality).? Contribute towards a multi-professional approach in the management of patientswithin the specialist area, especially in relation to ongoing care needs anddischarge arrangements.? Establish and maintain effective communication with patients and carers /relatives.? Support patients and their families and ensure that they receive the requiredinformation to enable them to self manage, lead or participate in their caredelivery.? Recognise changes in patients condition that require the intervention of othersand refer on as appropriate.? To support, assist and encourage the multi-disciplinary team in resolving patientproblems, by the provision of new and innovative models of case management.? To lead the specialist nursing contribution to the service, work in partnership withlead clinicians, General Managers, Matron to ensure the delivery of highstandards of patient care.? Work with the Matron to promote the development of nurses/AHP's undertakingthe CNO 'Ten Key Roles' as considered appropriate e.g.Prescribing., The Clinical Nurse/AHP Specialist will act as the clinical expert and professionalresource to the multi- professional team, patients and carers within theirSpeciality Service. The post holder will have a responsibility to develop the specialist area of practicein partnership with other members of the speciality team will work collaborativelyto develop services and improve the quality of care delivered. The post holder will carry responsibility for either a defined case load or offeringhighly specialist service advise this will include setting standards of care, theassessment of care needs and the development, implementation and evaluationof programmes of care, management and education. The post holder will contribute to achieving the objectives of the clinical servicegroup / speciality corporate objectives and work within the clinical governanceframework. This will be achieved through the core dimensions of the role, clinical practice,leadership, acting as the patient advocate and through audit and education.
