Full Job Description
Lead the operational delivery of our Transfer of Care Hub, driving safe, timely patient discharge and improving system flow across hospital and community partners. You will oversee the day-to-day running of the service, manage Transfer of Care and Discharge staff, and lead escalation processes to address discharge delays while ensuring robust reporting of discharge performance across the organisation and wider system. We are seeking an experienced and motivated operational leader to join our team as Clinical lead of the Transfer of Care Hub (ToCH). This is a key leadership role responsible for ensuring the effective coordination of discharge pathways and supporting the safe and timely discharge of patients from hospital. The post holder will lead the day-to-day operational management of the Transfer of Care Hub, working closely with multidisciplinary teams, hospital services, and community partners to reduce discharge delays and improve patient flow. The role requires strong leadership, system collaboration, and the ability to resolve complex discharge challenges while maintaining a patient-centred approach. You will play a critical role in ensuring that patients who no longer meet the criteria to reside in hospital are discharged safely and efficiently through the appropriate discharge pathways, supporting improved outcomes for patients and the wider health and care system.,
- Provide operational leadership for the Transfer of Care Hub.
- Lead and support the Transfer of Care and Discharge team, providing line management, professional leadership, and operational oversight.
- Coordinate the triage and management of referrals across discharge pathways, ensuring patients are placed on the most appropriate pathway.
- Develop and implement robust escalation processes to manage complex discharges and delays.
- Monitor discharge performance and provide regular reporting on discharge activity, delays, and outcomes to support service improvement.
- Support the management of complex cases and facilitate multidisciplinary discussions to enable safe and timely discharge.
- Contribute to the development and continuous improvement of discharge pathways and patient flow processes across the organisation.
- The post holder will demonstrate a strong understanding of the legislative and policy frameworks that underpin safe and effective hospital discharge.
- This includes a working knowledge of the Care Act 2014, the Health and Care Act 2022, and national Discharge to Assess (D2A) guidance.
- The successful candidate will understand the responsibilities of health and local authorities under the Care Act, including safeguarding duties, assessment of care and support needs, delayed discharge considerations, and collaborative working with local authorities and community partners to ensure safe and timely discharge arrangements for patients.
We are looking for a dynamic leader with strong operational and clinical understanding of discharge and patient flow. You will have experience of working across organisational and system boundaries, managing complex operational pressures, and driving improvements in discharge performance. You will bring excellent communication, problem-solving, and leadership skills, with the ability to influence and collaborate with a wide range of stakeholders to achieve safe and timely discharges for patients.
