Clinical Lead Occupational Therapist

We are seeking an enthusiastic Clinical Lead Occupational Therapist to join the Integrated Stroke Early Supported Discharge & Community Neuro Rehabilitation Service. The service provides both stroke rehabilitation & community neuro rehabilitation supporting service users with a variety of complex needs. This is an exciting opportunity to join an established multi-disciplinary team comprising of Occupational Therapists, Physiotherapists, Speech & Language Therapists, Psychologists, Specialist Nurses & Neuro Rehab assistants. This role requires the candidate to manage a clinical caseload across the South of Cambridgeshire. The base will be Brookfields - Cambridge or Hinchingbrooke Hospital - Huntingdon. The role involves visiting patients in their homes, wards or community gyms. There may be times when needed to provide clinical cover elsewhere within Cambridgeshire. The role requires agile working often using virtual consultation to manage a caseload. You will use your clinical skills and knowledge to shape the service, support others in their development. You will work closely with all members of the Multidisciplinary Team and assist the Locality Team Lead with service development. You will be involved as needed in the complaints process and serious incidents reviews. You will provide comprehensive assessments and deliver evidence-based specialist neuro occupational therapy programmes. You will work as part of a multidisciplinary team in supporting patients with physical, cognitive, and psychosocial needs to achieve their potential. • To have a clinical lead responsibility in the service, developing pathways in conjunction with locality and Service Leads.
• Represent and deputise for the locality team lead as required.
• To assess, plan and develop specialist OT programmes for patients with complex neurological problems and a wide variety of complex needs.
• Using expert knowledge, advanced clinical and autonomous decision-making skills, lead the team to manage patients with highly complex and sometimes unstable health needs.
• Ensure systems are in place to evidence the ward is compliant with care quality commission standards.
• To work as an autonomous specialist practitioner within the multi-professional Integrated Stroke ESD and Community Neuro Rehabilitation Service.
• To be responsible for own caseload and undertake specialist assessment of patients with complex and multiple pathologies using advanced, specialist clinical reasoning skills.
• To identify patient needs, agree goals and provide appropriate therapy interventions relating to their physical, functional, psychological and cognitive needs. Provide adaptive equipment, rehabilitation programmes, and work with other OT colleagues to develop recommendations for minor and major adaptations.
• To be responsible for referring the patient onto other services and agencies to ensure their identified needs are met.
• To keep the patient at the centre of care, ensuring patient and carer participation in decision making.
