Clinical Lead - Neighbourhood Mental Health Team, Waverley Locality

Based at BerkeleyHouse in Godalming, the WaverleyNeighbourhood Mental Health FRAILTY, LATER LIFE & DEMENTIA service provides comprehensive assessment, diagnosis, treatment, and ongoing support for individuals experiencing functional mental health conditions, frailty, cognitive decline, and/or dementia., 1.To support the service manager in the effective operation of the Frailty, Later Life and Dementia service line.2. To ensure the service operates in a safe, effective & efficient way as part of the whole Runnymede community neighbourhood team.3. To manage complex cases, providing leadership and supervision of staff and ensuring high quality evidence-based patient care.4. Being part of the community neighbourhood clinical leadership team -working collaboratively to ensure people's needs are met and a reduced need for repeat assessments.5. Assess patients/clients; plans, implements care; provides highly specialist advice; maintains associated records.6. May liaise with other agencies in planning programmes of care and/or health and education programmes
