Clinical Lead - Neighbourhood Mental Health Team, Runnymede Locality

Responsible for a defined complex caseload within the team. Using specialist Mental Health knowledge and skills, carry out individual clinical assessment, plan and formulate a range of interventions and treatment options within care plans, implement and evaluate packages of care.

Assess and treat clients with complex, severe and enduring mental health problems who may be on a community section of the mental health act.

Facilitate the transfer of client from Primary Care to Mental Health Services as appropriate and discharge clients from case load to appropriate agencies when clinical interventions are completed.

To be part of the Senior Staff Group and attend relevant regular meetings as scheduled.

Ensure information is collected in such a way that it conforms to the Trust's and Social Care quality data required.

To attend Tribunals and other Multi-Disciplinary Meetings and prepare reports.

To supervise and participate in the safe carriage, storage and delivery of medication to clients both in the clinical area and domestic environment, according to Trust and National Guidelines and Policies, ensuring that ordering and delivery of clinical equipment meets policy requirements.

To act as mentor or delegate appropriate mentor for clinicians from other clinical areas wishing to gain experience in Team.

To ensure that carers are in receipt of Carer's Assessments in accordance with the Carer's Act.
