Full Job Description
The post will be instrumental in the success and future developments of the pathway and will ensure that all professional and patient contact with the service is of the highest professional standard at all times. Working alongside senior clinical staff you will respond rapidly to requests for assistance when a young person is presenting in crisis.
You will contribute to the assessment and management of young people experiencing episodes of crisis within a clear and structured Risk Assessment and Risk Reduction framework and identify therapeutic goals to support the young person and families/carers.
You will refer on to specialist in-patient services as necessary or work collaboratively with other services involved to reduce the need for inpatient care. You will develop holistic, safety and recovery focused care plans and mobilise support around the child and family to promote resilience and reduce the likelihood of future crisis episodes.
As a member of the multi-disciplinary team the service will intensively support young people and families under a number of different pathways. This includes an in-reach, consultation and assessment model where a young person has been admitted to a paediatric ward, out of hours Crisis Access and Referral Point, rapid intervention on 72 hour care pathways and longer term intervention on 4-6 week Home Intensive Treatment care pathways., · To have management of complex caseloads and exercise the highest standards of autonomous professional responsibility for the assessment, treatment and discharge of children, young people and their families/carers using specialist knowledge and evaluation of care programmes in collaboration with CYP and their families/carers liaising with and referring to relevant agencies as appropriate.
· To undertake specialist assessment of CYP with mental health needs referred to the service including risk assessment, gathering appropriate information from their family/carers and other relevant agencies developing a formulation and plan for intervention, based on an appropriate conceptual framework in collaboration with the child, yp & parents/carers, communicating this to the referrer and other relevant professionals.
· To have highly developed specialist knowledge, underpinned by theory and practice that allows delivery of evidence-based interventions in line with NICE guidance & that meet the standards and requirements of the recommended psychological therapy.
To clinically supervise junior members of the team. Providing guidance & support to other clinicians, fostering a collaborative environment. Supporting developing staff training and learning for service delivery improvement.
