Full Job Description

Based in the Royal Bolton Hospital Clinical Haematology service, the post holder will provide expert, patient-centred nursing care to patients living with Sickle Cell Disease (SCD), supporting their physical, psychological and social needs. The post holder will act as a clinical specialist, educator and advocate, working collaboratively with multidisciplinary teams to optimise patient outcomes and enhance quality of life.,

  • Manage a defined caseload, including assessment, care planning, treatment, follow-up, and ongoing monitoring
  • Deliver advanced clinical care, including support during acute crises, chronic disease management, and emergency response where needed
  • Undertake nurse-led clinics and provide specialist Haematology treatments in line with agreed protocols and competence
  • Develop and implement individualised care plans in partnership with patients, families, and multidisciplinary team
  • Offer specialist advice, education, and emotional support to patients, carers, staff and students
  • Act as a specialist resource and advocate for Sickle Cell Disease with the Haematology service and wider Trust
  • Work closely with multidisciplinary teams and haemoglobinopathy network services to coordinate holistic care
  • Contribute to service development, including protocols, policies, patient pathways, and awareness of Sickle Cell Disease
  • Participate in audit, research, quality improvement, annual reviews, and service evaluations in line with HCC guidance
  • Maintain accurate clinical records and data systems and support evidence-based practice
  • Communicate highly complex and sensitive information effectively to patients, carers, and professional colleagues
  • Represent the service professionally through teaching, presentations and training activity
