Full Job Description
Based in the Royal Bolton Hospital Clinical Haematology service, the post holder will provide expert, patient-centred nursing care to patients living with Sickle Cell Disease (SCD), supporting their physical, psychological and social needs. The post holder will act as a clinical specialist, educator and advocate, working collaboratively with multidisciplinary teams to optimise patient outcomes and enhance quality of life.,
- Manage a defined caseload, including assessment, care planning, treatment, follow-up, and ongoing monitoring
- Deliver advanced clinical care, including support during acute crises, chronic disease management, and emergency response where needed
- Undertake nurse-led clinics and provide specialist Haematology treatments in line with agreed protocols and competence
- Develop and implement individualised care plans in partnership with patients, families, and multidisciplinary team
- Offer specialist advice, education, and emotional support to patients, carers, staff and students
- Act as a specialist resource and advocate for Sickle Cell Disease with the Haematology service and wider Trust
- Work closely with multidisciplinary teams and haemoglobinopathy network services to coordinate holistic care
- Contribute to service development, including protocols, policies, patient pathways, and awareness of Sickle Cell Disease
- Participate in audit, research, quality improvement, annual reviews, and service evaluations in line with HCC guidance
- Maintain accurate clinical records and data systems and support evidence-based practice
- Communicate highly complex and sensitive information effectively to patients, carers, and professional colleagues
- Represent the service professionally through teaching, presentations and training activity
