Would you like to opportunity of a brand new role working as a Clinical Governance & Risk Practitioner for the Directorate? This role involves working closely with CGRP colleagues in the Division and with colleagues across the Pharmacy Psychological Medicine & Sterile Services (PPMSS) Directorate. This role encompasses incident management, risk management, clinical audit and clinical effectiveness programmes. An excellent working knowledge of PSIRF and human factors and systems analysis is essential., The appointee will assist the Lead Governance Pharmacist to develop robust systems and processes in relation to clinical governance and quality, as required by CQC and NHSI or other regulators. The post holder works with front line staff to build engagement in key clinical governance processes and programmes and co-ordinate the delivery of the clinical governance and quality agenda. The post holder will deliver specific training e.g. local incident & audit management system, incident investigation, impact of human factors, risk and CQC self-assessment. A key role in the preparation of the Directorate monthly quality report, collation of information for the Directorate Risk Management report and incident investigation reports. The appointee will support the investigation and preparation of new incidents for discussion at the weekly Safety Learning & Improvement Conversation (SLIC) forum. The post holder has a key role in the collation and tracking of progress of the PSII investigation, Learning MDT, After Action Reviews and no harm incidents to closure, with proactive sharing of learning, as part of our commitment to becoming a high reliability Directorate., 1. Co-ordinate the implementation of a Directorate clinical governance programme linked to the Trust's overall clinical governance agenda. 2. Assemble and analyse quantitative and qualitative information (with divisional leads) in ways that assist staff in reflecting and improving on service delivery and practice. 3. In liaison with Divisional Resources (CGRPs and Divisional Lead pharmacists) assist with the compilation of relevant Directorate reports to assist with monitoring and enhancing its approach to the management of clinical quality & clinical safety and to enable the Directorate to provide appropriate assurance to the Trust in relation to clinical quality (through reports to the relevant Clinical, Governance, and Quality Committees). 4. Facilitate the downward dissemination of decisions and discussions at relevant Clinical Governance and Risk Committees and provide clarity regarding how they may relate to the individual Directorate (and CSU within the Directorate) and develop associated programmes to address and monitor the impact of the change. 5. In liaison with Directorate, Divisional and Corporate Resources assist in the building and development of information resources pertinent to clinical governance as a resource for the Directorate to develop improvement programmes to drive improved patient outcomes. 6. Facilitate and support directorate resources to co-ordinate, and report against the activities of Directorate quality and clinical governance groups 7. Assist the Directorate Resources (such as Dispensary Lead and Directorate Education Lead) to exercise his / her responsibility for risk management across the Division. 8. Support the ongoing use of the Trust incident reporting system within the Directorate, providing support in the investigation of incidents where appropriate. 9. Co-ordinate and support the management of PSII investigation, Learning MDT, After Action Reviews, Never Events or other incidents of moderate and above harm, within the Directorate in association with the Directorate resources and the Divisional clinical governance team. 10. Participate in management of an effective and appropriate Directorate risk register in liaison with Directorate resources for clinical governance. 11. Provide support to the Directorate to ensure that external inspections have appropriate action plans and that corrective actions have been completed with an associated follow up audit planned (as required). 12. Work alongside clinical audit and clinical governance leads within CSUs and services to develop and implement audit and improvement programmes that supports the needs of the organisation. 13. Work in consultation with Divisional/Directorate clinical leads to develop appropriate outcome measures, that are monitored and reported on an ongoing basis to assess and develop the quality of services provided. 14. Identify training needs within the Directorate (based, for example, on the analysis of incidents or the reports of relevant national audits) and work with the Lead Governance Pharmacist and Directorate Education Lead and divisional colleagues to ensure that these needs are met. 15. Undertake and provide clinical governance training sessions in the Directorate and where indicated across the organisation. 16. Support staff who are involved in incident investigations as part of supporting staff well-being and prevention of a second victim., The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
