Full Job Description
As a team member, you will provide senior clinical input to our community wards, working closely with the wider multidisciplinary team to deliver safe, person-centred care for older adults. The role offers hands-on clinical experience across the community inpatient setting in County Hospital supporting management of the older adult and discharge planning and supporting with collaborative decision-making with nursing, therapy, pharmacy and social care colleagues., The successful candidate will be a valuable and senior member of the Community COTE Directorate and will be responsible to the senior physician to whom they are working. The expectation of the role is to provide senior support as part of a multi-disciplinary team. The post holder will be expected to participate in outpatient clinics related to the specialty and take part in multidisciplinary meetings within the department. The post holder will carry out independent ward rounds with senior support available, where needed. The Senior Clinical Fellow post will involve providing active medical input to patients admitted to the ward for rehabilitation, stroke rehabilitation or palliative care needs. Oak Ward itself is a 29-bedded ward split across two floors/ the Senior Clinical Fellow will primarily be based on Upper Oak Ward but will be expected to provide clinical support to the lower floor dependent on need and at the discretion of the senior clinician. The post will work closely with Torfaen Community Resource Team and the wider nursing staff. Upper Oak Ward is staffed by a Specialist Grade and this Senior Clinical Fellow post, whilst the lower floor is supported by a Specialty Doctor with Consultant input., The main responsibilities of the role are for the effective and appropriate medical care of inpatients admitted under the care of the senior clinical lead. Duties and responsibilities will be commensurate to grade and specialty with appropriate supervision. These will include (but are not restricted to):
- The assessment, monitoring and initiation of treatment with colleagues and under supervision of the senior clinician
- Attending daily board rounds and regular ward MDTs
- Providing care for patients attending outpatient clinics
- Attending multi-disciplinary ward rounds as directed by the senior clinician
- Completing discharge summaries within 24 hours of discharge of all patients The post holder will generally be expected to undertake their programmed activities at their principal place of work or other locations as mutually agreed in the job plan. Exceptions will include travelling between work sites and attending official meetings away from the workplace. The post holder may be required to work at any site within the Health Board, including new sites, depending on service need.
