Full Job Description

1. To show dynamism, enthusiasm and flexibility in participating in the supporting and shaping of community and interface frailty services as part of SDHC.
2. To help provide a first class clinical service to all patients.
3. To provide high quality leadership and support to medical staff, nursing and allied health care professionals across the Homefirst Service.
4. To lead and support the multi-disciplinary team with complex discharge planning.
5. To support and sustain teaching and research where possible in relevant clinical areas.
6. When working in the community, the successful applicant will be expected to bring core medical skills; expertise and leadership to a wider team of clinicians tasked with the delivery of multidisciplinary assessment and integrated multi-agency interaction.
7. To work within the framework of applicable Trust policies, procedures, and clinical governance framework.
8. To work within contractual requirements, and in support of, applicable Trust clinical development frameworks, business plans, and development strategies.

Working for our organisation
Surrey Downs Health & Care and Epsom & St Helier University Hospitals Trust have developed an Integrated Virtual Ward model of Care which brings together primary care, secondary care and community services within Surrey Downs Place to support patients by offering acute level care, monitoring and treatment in their own homes. This offering is a safe and efficient alternative to NHS bedded care preventing avoidable admissions into hospital and supporting early discharge out of hospital. This @home virtual ward runs 7 days a week, 8am to 8pm. It is delivered by a multidisciplinary team including consultants, doctors, nurses, therapists, physician associates, paramedics, pharmacists, care assistants and administration. The team offers medical assessment, daily review, MDT discussion, and management of conditions.
The core function of this model is the @Home virtual ward which provides rapid assessment and wrap around care to people in their own home (usual place of residence) who become suddenly unwell and would normally be admitted to hospital or we can support with an earlier discharge from hospital., The principal duty of this post is to provide a comprehensive clinical service to patients.
+ To at all times, comply with the GMC's guidance on 'Good Medical Practice' as amended or substituted from time to time.
+ To ensure that patient confidentiality is maintained at all times.
+ To undertake duties as defined within the attached job plan in so far as practicable and in agreement with your Clinical Director.
+ To be responsible and managerially accountable for the reasonable and effective use of Trust resources that you use and influence, and professionally accountable to the Clinical Director.

For new entrants into the NHS, your pay under the Agenda for Change or Medical & Dental Pay Bands, will normally be set at the minimum of the pay band as advertised.
