Full Job Description

This is an exciting new position within Clinical Equipment advice team (based at Medequip). Providing local expertise to support the clinical management of the Community Equipment Service provision, supporting equipment prescribers from across multiple professions and organisations to deliver a highly effective community equipment service. The post holder will also contribute to development work relating to the wider Community Equipment Service, as determined by CDDFT Community Services and the Community Equipment Service Partnership Board, The service provides community equipment to adults in community settings and Children in both home and education settings across County Durham and Darlington.

The role is to support the clinical advice team in reviewing/authorising equipment orders, re-issuing special equipment and facilitation of community equipment training programme.

Provide a high level of support and advice in relation to equipment provision from therapeutic/nursing perspective including in-person assessments where required.

Supporting service developments within the community equipment service and undertake evidence-based audit to further own and teams clinical practice within the specialism.

To make recommendations to Clinical Lead of service that may lead to implementation of specific changes to practice or contribute to service protocols

The post holder will be an autonomous practitioner with the freedom to act. Being personally, professionally and legally accountable for the services provided to patients in their care and for those Associate Practitioners, Care and Therapy Support workers. The development and maintenance of close working relationships with the community equipment provider (currently Medequip) and equipment prescribers from multi agencies is central to the post.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
