Clinical Co-Ordinator Children's Palliative Care and Rapid Response

The Clinical Coordinator for Children's Palliative Care and Rapid Response provides clinical leadership,operational management and oversight of the service. The post holder ensures safe, responsive and high-quality care for children and young people with life-limiting, life-threatening and complex health needs, supporting care in their preferred setting.

Working with the Clinical Service Lead and Service Manager, the post holder coordinates clinical activity, staffing and resources to meet changing patient needs. The role combines expert clinical practice with leadership, supporting rapid response interventions, symptom management, end of life care and reducing avoidable hospital admissions through proactive assessment and care planning.

The post holder provides specialist advice and support to multidisciplinary teams, children, young people and families, promoting evidence-based practice, service development and high standards of governance, safeguarding and risk management.

Responsibilities include workforce coordination, service performance, staff development, supervision and education. Through partnership working with health, social care and voluntary sector providers, the Clinical Coordinator supports integrated, child-centred care that improves outcomes and patient experience.

The service operates seven days a week from 07:30 to 22:00, requiring flexible working to support urgent and complex care within community settings.

Main duties of the job

Provide clinical and professional leadership to the Children's Palliative Care and Rapid Response Team, ensuring safe, effective and high-quality care.

Coordinate daily service delivery, including planned and urgent visits, ensuring staffing and resources meet patient and family needs.

Undertake advanced assessment, care planning and review for children and young people with complex, life limiting and life threatening conditions.

Support symptom management, rapid response interventions and end of life care within community settings.

Lead virtual clinics and proactive care planning to improve outcomes and reduce avoidable hospital admissions.

Provide specialist advice and support to staff, children, young people and families, promoting evidence-based practice.

Work with health, social care, hospice and voluntary sector partners to deliver integrated, child-centred care.

Monitor service activity, quality and performance, identifying risks and leading service improvements.

Ensure safeguarding, clinical governance, risk management and professional standards are maintained.

Support workforce planning, recruitment, rota management and effective resource deployment.

Provide clinical supervision, mentorship, education and training to support staff development.

Contribute to policy, pathway and service development to improve quality and patient outcomes.

Act as an expert practitioner and senior clinical role model, demonstrating effective leadership, judgement and decision-making.
