Clinical Co-Ordinator Children's Palliative Care and Rapid Response

The Clinical Coordinator for Children's Palliative Care and Rapid Response provides clinical leadership, operationalmanagementandoversightoftheservice.Thepostholderensuressafe,responsiveandhigh-qualitycareforchildrenandyoungpeoplewithlife-limiting,life-threateningandcomplexhealthneeds,supportingcareintheirpreferredsetting.

Working with the Clinical Service Lead and Service Manager, the post holder coordinates clinical activity, staffing and resources tomeetchangingpatientneeds.Therolecombinesexpertclinicalpracticewithleadership,supportingrapidresponseinterventions,symptommanagement,endoflifecareandreducingavoidablehospitaladmissionsthroughproactiveassessmentandcareplanning.

Thepostholderprovidesspecialistadviceandsupporttomultidisciplinaryteams,children,youngpeopleandfamilies,promotingevidence-basedpractice,servicedevelopmentandhighstandardsofgovernance,safeguardingandriskmanagement.

Responsibilities include workforce coordination,serviceperformance,staffdevelopment,supervisionandeducation.Throughpartnershipworkingwithhealth,socialcareandvoluntarysectorproviders,theClinicalCoordinatorsupportsintegrated,child-centredcarethatimprovesoutcomesandpatientexperience.

Theserviceoperatessevendaysaweekfrom07:30to22:00,requiringflexibleworkingtosupporturgentandcomplexcarewithincommunitysettings.

ProvideclinicalandprofessionalleadershiptotheChildren'sPalliativeCareandRapidResponseTeam,ensuringsafe,effectiveandhigh-qualitycare.

Coordinatedailyservicedelivery,includingplannedandurgentvisits,ensuringstaffingandresourcesmeetpatientandfamilyneeds.

Undertakeadvancedassessment,careplanningandreviewforchildrenandyoungpeoplewithcomplex,life limitingandlife threateningconditions.

Supportsymptommanagement,rapidresponseinterventionsandendoflifecarewithincommunitysettings.

Leadvirtualclinicsandproactivecareplanningtoimproveoutcomesandreduceavoidablehospitaladmissions.

Providespecialistadviceandsupporttostaff,children,youngpeopleandfamilies,promotingevidence-basedpractice.

Workwithhealth,socialcare,hospiceandvoluntarysectorpartnerstodeliverintegrated,child-centredcare.

Monitorserviceactivity,qualityandperformance,identifyingrisksandleadingserviceimprovements.

Ensuresafeguarding,clinicalgovernance,riskmanagementandprofessionalstandardsaremaintained.

Supportworkforceplanning,recruitment,rotamanagementandeffectiveresourcedeployment.

Provideclinicalsupervision,mentorship,educationandtrainingtosupportstaffdevelopment.

Contributetopolicy,pathwayandservicedevelopmenttoimprovequalityandpatientoutcomes.

Actasanexpertpractitionerandseniorclinicalrolemodel,demonstratingeffectiveleadership,judgementanddecision-making.

We are seeking an exceptional Clinical Coordinator to join our Children's Palliative Care and Rapid Response Service. This is an exciting opportunity to play a pivotal role in leading and coordinating a dedicated multidisciplinary team that supports children and young people with life limiting, life threatening and complex health needs across Birmingham.

Working closely with the Clinical Lead and Service Manager, you will provide expert clinical leadership, coordinate service delivery and help shape the future of children's palliative and rapid response care.

About the Role

As Clinical Co-Ordinator, you will be responsible for the day to day operational coordination of the service, ensuring children and families receive responsive, safe and effective care when they need it most. You will support the allocation of staff and resources, provide expert clinical advice and leadership, and work collaboratively with acute, community and hospice partners to deliver seamless care.
