Clinical Care Home Nurse

Are you an experienced, motivated and compassionate Registered Nurse looking for an opportunity to lead, innovate and make a lasting difference within community and primary care?,

• Supporting end-of-life care planning and advance care discussions for the residents in our care homes and completion of ReSPECT forms to ensure their wishes are met at end of life.
• To lead in the weekly home rounds and have the ability and knowledge to make decisions regards appropriateness of onward referrals and hold others to account for their delivery of actions.
• A responsibility for ensuring that the residents and care home managers within Newark PCN locality receive assistance in accessing health care interventions by signposting to the most appropriate person to provide care.
  
  Significant post-registration experience within community, primary care, care home or older adult services.
• Experience of multidisciplinary team working.
• Strong assessment, care planning and clinical decision-making skills

We are seeking an experienced Registered Nurse to join our established and highly regarded care home team. This is an exciting opportunity to work across Newark's GP practices and care homes, supporting some of the most vulnerable people in our communities through the delivery of the Enhanced Health in Care Homes (EHCH) framework. As an experienced Registered Nurse, you will be working alongside a dedicated multidisciplinary team which is currently comprised of: 2x advanced nurse practitioners, 1x clinical pharmacist, 3x nursing associates and 1x care co-ordinator. You will help drive improvements in quality of care, resident experience and clinical outcomes, while supporting people to live healthier, safer and more independent lives.
