Chronic Disease Nurse

If so, come and join us!, We have a 8 sessions/30 hours per week vacancy for a Chronic Disease trained Nurse to join our thriving, democratic and enthusiastic team at our surgery located in Redditch Town Centre. Redditch is located 15 miles south of Birmingham within easy reach of Stratford, Worcester and surrounding countryside with good transport links and an increasing and diverse population., Undertake a range of nursing assessments and provide appropriate care / treatment in conjunction with GPs according to policy and protocols and adhere to the Practice policies, protocols and guidelines at all times.

Strive to improve standards of care and be involved in identifying practice population needs.

Knowledge of the GMS Contract is an essential part of the role as well as the mentoring of our Health Care Assistants to assist with achieving the GMS Contract needs.

To maintain a safe and effective level of practice and to bring any deficits in training to the attention of the Practice Manager so that these can be addressed.
