Full Job Description

This post is designed to facilitate early discharge and prevent avoidable readmission to hospital for children with either acute or chronic health care needs, through the provision of specialist support from an autonomous practitioner to their family or carers. The seamless transition from hospital to home will be achieved through multi - professional / multi - agency working, across all health care settings. To protect, safe-guard and promote the well-being of children in accordance with 'The Children Act, 1989' and 'Working Together Under the Children Act, 1989'., CLINICAL: 1. Undertake assessments of care needs within the community. Act in partnership with the child and family when assessing a child's needs and planning, 2 implementing and evaluating care in accordance with the children's community nursing service philosophy of care. 2. Assess, treat, evaluate and manage the care of children with various conditions 3. Recognize and monitor acute exacerbations of illness and initiate appropriate nursing interventions prior to senior clinical input being available. 4. Discuss children on their case-load with designated senior team members 5. Reassess care needs and care outcomes through ongoing discussion and negotiation with families / carers. 6. Provide support to families / carers to enable them to become confident in their ability to meet their child's health care needs at home. 7. Act as an expert resource to the child and family in their own home. 8. Collaborate with the multidisciplinary team and other service providers to ensure care is patient centered, efficient and effective. 9. To be involved in health education/promotion for children, families and carers with guidance from a senior member of the team. 10. Maintain precise documentation relating to patient care according to the NMC standards for record keeping and provide such statistics and information as required by management. 11. Act as a role model and a resource / advisor to other professionals within the trust and the primary health care team. 12. Maintain clinical expertise and develop research based practice skills, within the scope of professional practice. Accessing specific / relevant training to enhance clinical skills where appropriate. 3 13. Be conversant with the procedures and information within the Lancashire Safeguarding arena PROFESSIONAL: 1. Maintain active status on the NMC register 2. Act always in accordance with NMC Code of Conduct and guiding documents. 3. Maintain up to date skills, knowledge and professional portfolio and be aware of current professional issues. 4. Evaluates own practice against the KSF Outline for the post and meets the requirements of the KSF Review Process. 5. Maintain legible, accurate records and documentation and monitor the effectiveness of the established communication / reporting system, in compliance with the NMC Code of Conduct. 6. Follow agreed policies and procedures of the trust and promote the Quality Assurance Initiative. 7. Raise the profile of the service by good communication and public relations. 8. Attend regular team meetings in accordance with mandatory requirements. MANAGEMENT: 1. Manage a caseload of children with guidance and support from a senior member of the team. 2. Maintain effective communication channels with General Practitioners, Health Visitors, District Nurses, School nurses, Regional Nurse Specialists, Hospital Professionals, Patients and family and liaise with other agencies as appropriate. 4 3. Be familiar with statutory and voluntary support services and agencies available to families with sick children. 4. Participate in quality assurance initiatives working towards achieving the highest standard of care. 5. Support the team leader in effective resource management. 6. Monitor and maintain equipment used by the CCN service 7. Be involved in the orientation, induction and supervision of junior staff 8. Ensure patient confidentiality is maintained at all times. 9. Advise clients on the complaints procedure and participate in official investigations when necessary. 10. Participate in audits of practice considering user views. 11. Seek support and advice in matters beyond own sphere of knowledge and experience. TEACHING: 1. Assist in ensuring that the learning environment complies with the NMC and UCLAN requirements. 2. Participate in supporting, mentoring and the assessment of learners on the community hub and spoke placement. 3. Comply with the trust's training strategy. 4. Comply with the directorates and trust's business plans. HEALTH AND SAFETY: 1. Comply with safety instructions and policies laid down. 5 2. Uses, in a proper and safe manner the equipment and facilities provided and report as practical any hazards and defects to a senior member of the team and/or the relevant department. 3. Refrains from the willful misuse or interference with anything provided in the interest of health and safety and any action which might endanger yourself and others. 4. Reports to senior staff as soon as practical any accidents on untoward occurrences, ensuring that incident forms are completed. 5. To engage with children, families and carers proactively in avoiding and resolving complaints. STANDARDS OF CONDUCT: 1. Ensure that personal actions and conduct comply with the Trust Policies and Procedures e.g. Health and Safety, Tobacco Control Policy, Equal Opportunities, Confidentiality, the Data Protection Act (1998) and Moving and Handling Regulations. 2. To promote and safeguard the wellbeing of children in accordance with 'The Children Act 1989' and 'Working Together Under the Children Act 1989' 3. Attend mandatory training courses as required e.g. induction, moving and handling, fire lectures etc. 4. Conduct duties with regard to values underpinning the Trust's Vision:

    This is an exciting opportunity to join a small, friendly, enthusiastic team of children's community nurses who care for sick children within their own home.
  • Following the discharge of children from hospital the team enables facilitation of a seamless service to the home setting whilst preventing unnecessary readmission. We are looking for a person with excellent clinical skills and diverse experience at Band 5 either in acute paediatrics or acute community setting who will provide a high standard of evidence based nursing care. An ability to work holistically with children and their families to ensure their care needs are met is essential. You will need to be an autonomous and flexible practitioner within the community setting, be a good team player and be able to establish effective collaborative working relationships with other, health care professionals, services and agencies. Recent experience of working in the community setting would be an advantage although not essential. If you enjoy a challenge you'll be joining an ever evolving Children's Community Nursing service, this will provide the successful candidate with an exciting opportunity to be involved in this process.,
