The CCN service aims to provide a high-quality nursing service which enables early, safe planned discharge from hospital and prevent unnecessary admissions providing a holistic approach to care within the community. With the support of the Senior CCNs and the Clinical Team Lead, you will act as a resource of paediatric expertise, providing advice to other professionals, promoting evidence-based practice. This will require following NMC guidelines, demonstrating autonomy, professional curiosity, and accountability.Safeguarding is a fundamental part of the CCN role and requires a sound knowledge base to safely manage their caseload.
You will be supported with regular supervisions, 1:1s and appraisals.
Our service is a link between Primary, Secondary and Tertiary care, and the wider MDT. Care is delivered in the community location that is the most appropriate for that child or young person.
If you have excellent communication and interpersonal skills, the ability to work as a team player, as well as the initiative to work autonomously, we would like to hear from you. You should have good organisational skills and evidence of continued professional development. Previous community nursing experience would be desirable, although not essential.
Our established passionate and dedicated CCN team work collaboratively with babies, children, young people, families, BCYP, and the wider multi-disciplinary team to ensure care is provided as close to home as possible. We provide high quality and holistic, family centred care to BCYP with a nursing need across the Southampton and West Hampshire area., Hampshire and Isle of Wight Healthcare NHS Foundation Trust provides joined-up mental and physical healthcare for around two million people across our communities.
