Full Job Description
Make a real difference to the lives of children and young people in your community. An exciting opportunity has arisen to join the Children's Continuing Care Team at Great Western Hospitals NHS Foundation Trust, supporting children and young people with complex medical needs to live safely and happily within their home and school environments. As a valued member of our dedicated and compassionate team, you will provide direct clinical care and support tailored to the individual needs of the children and families we serve. Your role may involve providing overnight care in a child's home, enabling families to rest and recharge, or supporting a child within their educational setting, helping them access learning and reach their full potential. We are looking for caring, enthusiastic and reliable individuals who are passionate about delivering high-quality care. Following a comprehensive competency-based training programme, you will be confident working independently as a lone practitioner within the community, while remaining fully supported by our experienced and friendly team. Please note: Part-time or term-time hours do not meet the required salary threshold for sponsorship and therefore would not be eligible for sponsorship
