An exciting opportunity has arisen for motivated individual to work within the Community Nursing Division as a Community Nursing Caseload Manager, within the Wallasey Locality working with The New Brighton community nursing team for maternity leave cover. There has never been a better time to join Wirral Community Health and Care Trust, as we have just received a CQC rating of GOOD with areas in OUTSTANDING. We provide Community Nursing Services over 24 hours that keep our local communities thriving - this is the heart of everything we do., 22.5 hours per week, fixed term for 10 months maternity leave cover. Hours and shift patterns to be discussed at interview.

This post will be responsible for managing planned programmes of care for patients in domiciliary and clinic settings.

A main part of the role of a Caseload Manager is to assess, plan, implement and evaluate nursing care to patients across the community/primary care interface in the most efficient and clinically effective way, in co-operation with other members of the health and social care teams, and voluntary and statutory agencies.

Our Community Nursing Team supports the needs of housebound patients living on the Wirral. We provide valuable care, supporting people to manage their long term conditions, and maximise their independence, which in turn can prevent admission into hospital or enable a quicker discharge if they admitted.
