Full Job Description

As Case Manager you can make your own powerful contribution to the lives of people with learning disabilities. Making a big difference for service users with complex needs, and for their families, you'll be an enthusiastic champion of independence, dignity, choice, self-esteem and wellbeing.

As a member of a specialist multidisciplinary team, you'll work closely with fellow case managers and social workers, under the guidance of managers with specialist experience in learning disabilities. You will relish the fulfilling challenge of working closely with those in complex circumstances and dealing smartly with budgets to deliver the best possible support.

Whether individuals live in the community independently, with family carers or in supported accommodation, you will take their unique strengths and levels of risk into account when carrying out assessments. We will look to you to take on the role of lead enquiry officer as part of the safeguarding process. You'll produce person-centred support plans and enable people and their carers to make informed choices about care services.

In return for your commitment you'll enjoy excellent opportunities for learning and career development.

To join us you'll need a recognised social work, qualification, plus membership of a professional register. With experience of working in a local authority or health setting, you demonstrate knowledge of relevant legislation and a commitment to the principles of personalisation, independence and choice. Ideally, you will have experience of working with people who have learning disabilities. You will certainly be used to working in a social services duty system as part of a multi-disciplinary team. An understanding of budgets and financial controls is also essential.

Your skills will enable you to start making a difference on day one - from carrying out accurate and timely assessments and promoting self-directed care, through to negotiating with other agencies and keeping comprehensive case records. Your ability to communicate, in person and in writing, with service users, cares and colleagues, really sets you apart.

THE EXTRAORDINARY STORY OF GIRO'S PERSUASIVE POWERS

The Learning Disabilities Team in Westminster City Council is a world of extraordinary stories. We love it when our work touches lives.

Take Stepping to the Beat, the Council's 3k walk, jog or run for people with learning or physical disabilities or Autism, their families and carers. It's a life-affirming, music-based event Coreen and Claudia are massively proud of - and they were keen for people to spread the word. So, imagine how thrilled they were when one of our residents, Giro, did just that. Advocating for people with disabilities at a planning meeting for transport in London, he was proudly wearing his event medal. And he decided to use it to encourage others. As he passed it around the room, Giro inspired lots more people with disabilities to get involved. Infectious enthusiasm goes a long way here., Westminster is an amazing place. We are home to more than 200,000 residents from all backgrounds, over 50,000 vibrant and vital businesses and three-quarters-of-a-million people work in Westminster. Westminster City Council's strategy is to work towards a Fairer Westminster. A Fairer Westminster is one in which our communities are at the heart of our decision-making and help to determine the city's future. By working directly with our communities and other partners, we can build a more inclusive city that celebrates our diverse communities, and where residents, workers and visitors from all backgrounds will feel welcome and safe.

At Westminster City Council, we pride ourselves on being an inclusive workplace and employer of choice. We encourage and welcome applications from people from all backgrounds and aim to have a workforce that, at all levels, represents the communities we serve. We champion equality, diversity, inclusion and wellbeing and aim to create a workplace where everyone feels valued, has a sense of belonging and is empowered to be their best, that is the Westminster Way.
