Full Job Description
Working with a community nursing team, you'll deliver high-quality care that helps people stay safe and well at home. You'll play a key role in supporting people with long-term conditions, coordinating care and reducing avoidable hospital admissions. You'll work closely with colleagues across health and care services to plan and deliver personalised care. This role makes a real difference by supporting patients and carers to manage their health and maintain independence., What you'll do:
- Assess patient needs and plan, deliver and review care in the community
- Coordinate care across services, acting as a key contact for patients and carers
- Carry out clinical procedures such as wound care and long-term condition management
- Maintain clear and accurate Electronic Patient Records (EPR)
- Support team planning and help prioritise workloads
Registered nurse with relevant experience in community or long-term condition care - Knowledge of care planning, assessment and evidence-based practice
- Confidence working both independently and within a multidisciplinary team
- Strong communication skills to support patients, carers and colleagues
