At Lincolnshire PCN Alliance (LPCNA) we are instrumental in shaping the future of Primary care across the county. We are here to innovate and develop lasting change across services, with a focus on improving outcomes through population health management, service transformation and strong partnership working. Collaboration is at the heart of everything we do. LPCNA brings together 14 Primary Care Networks, representing 81 practices and serving over 824,000 patients, to create collective impact across the wider health and care system. This includes supporting the development of neighbourhood-based models of care and bringing teams together across organisational boundaries to provide more joined-up, accessible services. Our leadership team is made up of experienced healthcare professionals and strategic thinkers who are passionate about transforming care. Drawing on expertise in general practice, healthcare policy, workforce development, project management, service transformation and integrated care systems, they help drive innovation, strengthen partnerships and improve outcomes for patients, practices and communities. We are entering an exciting period of growth and are looking for enthusiastic, values-driven individuals who share our ambition to improve population health and shape the future of healthcare across Lincolnshire.
