Full Job Description

We are recruiting for a Care Navigator who will be the first point of contact for patients, helping them access the right GP or healthcare service. The role involves answering calls and enquiries, booking appointments, handling patient information, making referrals, and providing friendly and professional support while maintaining confidentiality.,

  • Being the first point of contact for patients.
  • Handling telephone, face-to-face and electronic enquiries.
  • Booking and arranging patient appointments/clinics.
  • Liaising with GPs, clinicians, administrative staff and patients.
  • Contacting patients with information from the clinical team.
  • Helping patients access GP services and appropriate services in the wider community.
  • Referring patients where appropriate.
  • Filing, coding and general administrative work.
  • Working with practice procedures and processes.
  • Providing a friendly, professional and compassionate service to patients.

    Carlisle Central Practice is an NHS GP practice serving patients across Carlisle and the surrounding areas. It provides a wide range of primary care services, including GP and nurse appointments, health checks, vaccinations and management of long-term conditions.
