Full Job Description

You will be a key point of contact for patients, helping them access the most appropriate healthcare professional or service. Duties will include booking and coordinating appointments, signposting patients to NHS and community services, updating patient records and working closely with clinical and administrative colleagues., You will support patients as a first point of contact, helping them access the most appropriate healthcare services and appointments. You will work closely with the practice team to ensure patients receive a helpful, efficient and welcoming service., Job Summary The Care Navigator is a key member of the GP practice team, responsible for helping patients access the most appropriate services for their needs. They support patient flow by directing requests to the right clinician or service within the practice, PCN, or external organisations. Key Responsibilities Act as first point of contact for patients (phone, face-to-face, online) Triage patient requests and direct to appropriate clinician or service Support appointment booking and manage patient queries Signpost to NHS and community services Work with GPs and multidisciplinary team to support care pathways Maintain accurate patient records using clinical systems (EMIS/SystmOne) Ensure GDPR and confidentiality compliance Support practice demand management and patient engagement Skills and Experience

Previous healthcare or GP practice experience is desirable but not essential., Strong communication and customer service skills Ability to handle sensitive conversations with empathy Good IT skills (clinical systems desirable) Ability to prioritise workload in a busy environment Understanding of NHS pathways desirable Experience in GP practice or healthcare setting desirable Personal Attributes Patient-focused and compassionate Calm under pressure Professional and reliable Team player Committed to confidentiality and safeguarding Purpose of the Role To ensure patients receive the right care from the right professional at the right time, improving access to GP services and supporting efficient use of NHS resources.
