Full Job Description
We have an exciting new vacancy for a care navigator. You would be the first point of contact for patients, helping to manage their journey through the healthcare system by coordinating services and providing administrative and reception support., Key responsibilities include answering patient queries, booking appointments, directing patients to the appropriate healthcare professional or service, and providing administrative support such as workflow duties. Care navigators also signpost patients to other community or voluntary services that could benefit their quality of life, such as mental health support, financial advice, or local support networks.
