Full Job Description
Care Home Trusted Assessor based at Northampton General Hospital supporting safe, timely and effective discharge of patients into residential and nursing care settings. Working as part of the National Care Home Trusted Assessor Programme, you will complete assessments, liaise with hospital teams, care homes, residents and families, identify suitable placements, and help reduce delayed hospital discharges while improving outcomes for older people.
