Full Job Description
- Administer medication and assist with clinical tasks under supervision
- Support the care planning process and liaise with external professionals
- Lead shifts and provide guidance to care assistants
- Promote person-centred care while maintaining residents' dignity and independence
- Champion our values and help create a warm, respectful environment for residents and team members alike, As a CHAP, you'll support the delivery of clinical care, acting as a link between our care assistants and nursing team. You'll be trusted to manage tasks such as administering medication, leading shifts, and supporting care planning - always with residents' dignity and wellbeing at heart.
A caring, confident, and proactive approach - Strong communication and teamwork skills
- Previous experience in care with willingness to progress
- NVQ Level 3 in Health & Social Care (or working towards it) preferred
- Medication administration training desirable
