Full Job Description
The Care Coordinator provides a patient-centered coordination service, supporting patients to access appropriate health, wellbeing, social and community services. The role involves working collaboratively with clinical and non-clinical teams to identify patient needs, coordinate appropriate support, improve engagement with healthcare and contribute to reducing health inequalities.
The role combines care coordination, patient support, health promotion, community engagement, patient education, outreach, administration, and service development.
A key aspect of the role is identifying gaps in patient support and developing innovative initiatives, educational activities and community-based opportunities that respond to identified health needs and encourage patients to become more engaged in managing their health and wellbeing.
Main duties of the job
1. Works within the primary care team, contributing to leadership of service evaluation and research to promote quality improvement activity.
2. Works with the Practices Patient Participation Group to support communication with the wider patient community.
3. Collaborates with other members of the team, patients and their carers when managing and coordinating care.
4. Uses healthcare technologies to optimise service delivery, patients access, and continuity of care, Through the Additional Role Reimbursement Scheme (ARRS) the PCN has expanded traditional GP services by employing individuals to undertake various roles in order to provide more proactive, personalised and integrated care for patients registered at the four practices. Our ARRS team consists of Care Coordinators, Physiotherapists, Dietitian, Pharmacists, Pharmacy Technicians and Social Prescribers, as well as our leadership team which includes two Clinical Directors, PCN Manager, and Digitial & Transformation Lead.
Havering Marshalls PCN consists of four GP practices in Romford, totaling approximately 50,000 patients
