We're looking for a Care Coordinator at our Stourport-on-Severn who enjoys organising, problem-solving, and connecting the right carers with the clients who need them most.The role of a Care Coordinator Coordinate care services and client schedulesMatch carers with clients to meet individual needsSupport the setup of new care packagesPlan rotas to ensure continuity of careCommunicate with families and healthcare professionalsRecord staff availability, absences, and emergency callsParticipate in the on-call rota when requiredWillingness to cover the on-call phone (additional pay provided) Every Other WeekendAbout you: We're looking for someone who is organised, people-focused and enjoys keeping things running smoothly, as well as:Strong organisational skills and attention to detailAbility to work confidently in a busy, fast-paced environmentFull UK driving licence and access to your own vehicleMinimum of 12 months remaining on your right to work in the UKWhat we offer: Refer-a-friend bonus of up to £1,000
