Our ethos is simple and powerful: Home Care With Love - for our clients and our colleagues alike., As a Care Coordinator, you'll be the key link between clients, families, and care workers. Reporting to the Branch Manager, you'll ensure care is delivered smoothly, safely, and consistently - while supporting and developing our frontline teams.
Your Key Responsibilities
Create and manage staff rotas, ensuring appropriate travel time and breaks
Match carers to clients based on skills, preferences, and location
Manage new referrals from private clients and social services
Support assessments, care planning, and care package reviews
Communicate with clients, families, and healthcare professionals
Carry out risk assessments, reviews, and care worker supervisions
Ensure care documentation and client records are accurate and up to date
Support weekly KPIs, logbook drops, and team meetings
Organise and participate in supervisions, appraisals, and training
Take part in the on-call rota, including alternate weekends
Competitive salary: £30,000 per annum
