Full Job Description
The PCN teams work with a wide range of partner organisations and stakeholders, including acute trusts, mental health trusts, local authorities, community services and primary care, as well as voluntary sector organisations. The PCN approach is to deliver integrated care based around the needs of service users and patients rather than being constrained by historical structures. We are looking to appoint Health and Social Care Coordinators who will play a key co-ordinating role at the heart of the GP Practices. The post holder will ensure that our patients receive the co-ordinated care they depend on, care, which is responsive to their needs and designed in cooperation with patients, carers and all supporting services. About us Our organisation is a well-established primary care practice supported by a multidisciplinary and experienced team. The practice has three GP partners and seven salaried GPs, providing a strong clinical leadership structure and continuity of care for our patient population. Our wider clinical team includes an ARRS (Additional Roles Reimbursement Scheme) multidisciplinary workforce, comprising clinical pharmacists, paramedics and a physician associate. This diverse team enables us to provide a broad range of skills and expertise within primary care, supporting the GPs and ensuring patients are seen by the most appropriate healthcare professional for their needs. The practice is supported by a Practice Manager, Deputy Practice Manager and a Reception Manager alongside a dedicated administrative and reception team., Working with colleagues, to identify high-risk patients and ensure they receive a care planning consultation with their GP. -Act as the first port of call for service users/patients, in their caseload in relation to their care. - Monitor referrals to ensure tasks are completed and care delivered by keeping in regular telephone contact. - To provide admin support to practices to ensure consistent reporting standards. - Support Quality and Outcome Frameworks and other DES/LES specifications. - Maintain and develop engagement with all practice staff and encourage 'best practice.'
