Full Job Description
We are looking for a highly efficient and solutions-focused Care Coordinator to join our friendly team in the beautiful village of Godstone. We have two positions available, with full-time and part-time (24-30 hours) working options available., The main responsibilities of this role are the close management of patients with long-term conditions, the planning and delivery of vaccination and screening programmes and other ad hoc initiatives such as catch-up programmes. Data analysis is key as the successful candidate will be responsible for running time-sensitive reports to ensure efficient recall processes are followed.
