Full Job Description

The Rotherham Assertive Outreach Team (AOT) provides intensive, recovery-focusedcommunity support to adults across Rotherham with severe and enduring mental illnesses,primarily psychosis. The service works with patients who often present with co-existingsubstance misuse, complex risks, and significant social challenges, many of whom havestruggled to engage with traditional mental health services. AOT aims to reduce barriers tocare, prevent hospital admissions, and promote rehabilitation through a highly integrated,multidisciplinary, and multi-agency 'team' approach in line with the AOT model. The servicealso includes a community rehabilitation pathway for patients with both psychotic and nonpsychotic disorders (SMI), and a 10-bedded supported living facility offering structured,step-down rehabilitative support in partnership with South Yorkshire Housing.The post-holder will deliver assertive and therapeutic interventions aimed at reducingrelapse and hospital admissions. Using DIALOG/DIALOG+ and other patients reportedoutcome measures, the Care Coordinator collaboratively assesses, develops and reviewspersonalised care and recovery plans, ensuring that support is holistic, person-centred, anddelivered in partnership with the wider multidisciplinary team and external agencies., Deliver community-based assertive outreach and rehabilitation supportive engagement, promoting recovery, independence, and social inclusion.o Manage a defined caseload of patients in the community and supported living, including patients subject to Community Treatment Orders and act as Social Supervisor for patients subject to Ministry of Justice sections.o Act as shift coordinator, planning daily visits and care delivery, responding to service demand and crisis situations.o Work collaboratively with the multidisciplinary team to manage complex risks.
