Full Job Description

An exciting opportunity has arisen for the recruitment of a Care Co-ordinator within the Medicine Division. The post holder will be an integral member of the team responsible for managing and facilitating patient discharges from the medical wards. The shift pattern will be Monday-Friday, core office hours and on a rotational basis there will be the requirement to work at a weekend.
The successful post holder will work closely with ward staff, divisional operational teams, patient flow and Discharge Planning within West Suffolk Foundation Trust as well as wider social services department's
