Full Job Description
We are currently seeking to recruit Care Co-ordinators (Level 1) to join our dedicated Mental Health Team. This rewarding role involves providing exceptional, person-centred care and support to one young female customer, ensuring that her individual needs, choices, and aspirations remain at the heart of everything we do. The successful candidate will support the customer to maintain and build a fulfilling lifestyle, promoting independence, dignity, respect, engagement, and participation in all aspects of daily living. You will work closely with her to achieve positive outcomes, encourage personal development, and ensure her health, wellbeing, and quality of life are always prioritised. We are looking for someone who is compassionate, reliable, and committed to delivering high standards of personalised care and support. You should be able to build positive relationships, communicate effectively, and demonstrate integrity while upholding Sunderland Care and Support's values at all times. Candidates should be aware that their are living dogs on the property. You will share our core values of Respect, Innovation, Integrity, Caring, Partnership and Being Valued, ensuring these are reflected in everything you do. Your role will be on providing support to customers promoting and developing their life, health, and wellbeing. This may include:
- Helping with preparation of meals and drinks.
- Support with domestic household tasks
- Administer and record medication
- Support with money management
- Encouraging community trips and facilitating customers participation
- Support with household and personal shopping,
- Support with meeting friends and family
- Support with high standards of nutrition
- Adhere to health and safety legislation
- Cleanliness and intimate personal hygiene
- Opportunities for employment or education
- Provide emotional support.
- Active support techniques and positive behaviour support techniques to increase skill level and achieve set outcomes (training will be provided)
- To support customer to plan holidays and support them if needed (additional payment) These principal responsibilities are not an exhaustive list of duties, only a general guide to the role. In consultation with the post holder, the duties may change from time to time to reflect the changing needs of the customer/service.
- Induction which will support you in your own personal development.
- We can offer 37 hours per week
- We pay for your DBS check
- A starting salary of £27,274 - £29,071 (pro rata if working less than 37 hours per week)
- Sleep in payment of £43.12 per shift completed. (Where applicable)
- Designated rota pattern including daytime, evenings, weekends, and Bank Holidays
