Full Job Description
We are seeking a Cardiac Rehabilitation Specialist Nurse to join our dynamic multi-disciplinary cardiac rehabilitation team. We are looking for a dynamic, caring and compassionate nurse with excellent communication skills, an ability to work independently, demonstrate evidence of professional development and a keen interest in health promotion. This post requires you to have the motivation and passion to participate in the development and delivery of the hospital, community setting, virtual and home-based exercise programmes. There will be a requirement to travel within Cambridgeshire to our out-reach setting in Cambourne. This post is ideal for a suitably skilled and trained cardiac nurse who is keen to develop as a cardiac rehabilitation nurse. You must be registered with the nursing and midwifery council. You are advised to submit your application as early as possible to avoid disappointment" Main duties of the job To provide education and support to patients post cardiac event in both in-patient and out-patient setting. To use clinical knowledge / expertise to provide a high standard of skilled care to patients. To manage own patient workload, prioritising needs and escalating as required. To be able to effectively plan, implement and evaluate the rehabilitation care provided to patients based on their individual assessment. To line manage the administration staff within the team. To be part of a multi-disciplinary team who deliver cardiac rehabilitation in the hospital, community, virtual and home-based exercise programmes, this may involve individual or group work of education and exercise. We will provide support and teaching to enable you to provide excellent patient care across all areas of cardiac rehabilitation. - Professional adult nursing qualification and clinical knowledge acquired through training to diploma or degree level
- Plus post graduate training in specialist area OR specialist knowledge across the range of work procedures and practices, underpinned by theoretical knowledge or relevant practical experience.
- Leadership/management training and/or experience.
- Registered with Nursing and Midwifery Council. Experience Essential
- Significant experience in field of own practice.
- Experience of managing/supervising multi-disciplinary team and ability to mentor/educate junior staff.
- An understanding of clinical governance frameworks including Patient Safety Incident Response Framework (PSIRF), policy and procedure, audit and quality assurance.
- An understanding of safeguarding policies and procedures, particularly those related to vulnerable patients.
- Excellent communication and interpersonal skills with the ability to be able to problem solve, use own initiative and plan, organise and prioritise own and others' workload.
- Evidence of continuous professional development and a willingness to participate in further training and education.
