Cardiac Nurse Specialist

To work as a specialist nurse in Cardiology clinics, working as part of the MDT to provide assessment and treatment planning. This role will support the delivery of Atrial Fibrillation, Valve and Rapid access chest pain clinics.

You will demonstrate an extensive cardiac knowledge with competence in assessment and consultation and registered prescriber as well as the ability to work independent and autonomously.

Provide care for patients in a holistic manner, incorporating physical and psychological care to enable them to live with a cardiac condition.

To provide education and support to the cardiology patients.

To be the point of contact for patients in the CNS clinics to give ongoing support virtually and to assess and liaise with other members of the MDT on their behalf.

To work alongside the MDT to deliver the highest standard of cardiac care and liaising closely with community teams to provide a joined up and effective service delivery. Engage with the MDT.

This is a fixed term position until 31st August 2027

Manages clinical risk taking responsibility for the assessment of patient needs and the development, planning, implementation and evaluation of care within the ward and post discharge.

Draws on specialist knowledge to act as an autonomous member of the CNS team, and provide specialised management and care of the cardiac patient.

Effectively deliver a high standard of care to the cardiac patient.

Carefully documenting and sharing information with the MDT in a useful and constructive way. Using methods available to communicate effectively with other health professionals including GP, community nurse specialists and consultants.

To be able to communicate effectively with the consultants and colleagues if there is a patient concern and understand the correct escalation process.
