Full Job Description

The OAFU (Open Access Follow Up) CSW (Cancer Support Worker) is responsible for delivering high-quality, compassionate, and patient-centred care to individuals following cancer treatment atImperial College Healthcare NHS Trust. The role supports patients on stratified follow-up pathways, with a strong emphasis on health promotion, self-management, and holistic wellbeing. Working collaboratively with multidisciplinary teams, cancer CNS teams, Macmillan services, and primary care providers, the post holder will play a key role in supporting cancer patients after treatment. The role focuses on co-ordinating follow-up care, monitoring patient progress and acting as a central point of contact for patients, carers, GP's and other multidisciplinary teams. The successful candidate will contribute to supporting patients to self-manage their condition, arranging appointments and investigations, monitoring patient progress through remote surveillance pathways and ensuring effective communication between multidisciplinary teams. They will provide telephone support and guidance and contribute to service development through participation in audits, quality improvement initiatives, and education of support staff and students.,

  • Coordinating remote monitoring and surveillance for patients on stratified follow-up pathways.
  • Supporting patients to self-manage their condition and recovery.
  • Acting as a single point of contact for patients and carers after treatment.
  • Performing telephone triage and escalating concerns appropriately.
  • Assisting with tracking and monitoring of follow-up patients.
  • Helping patients navigate health and social care services.
  • Working closely with the Multi-Disciplinary Team (MDT).
  • Organising appointments, investigations, and follow-up results.
  • Providing telephone advice, triage, and signposting patients to appropriate services.
  • Ensuring accurate documentation and record keeping.
  • Supporting access to wellbeing services, support groups, and charities.
