Full Job Description

Fixed Term Contract or Secondment for 9 months, full time 37.5 hours per week.
An exciting opportunity has arisen for a fixed term contract or secondment as a Cancer Pathway Navigator within the Colorectal Team, to work with the Colorectal Triage and Colorectal Nurse Specialist .
The post holder will act as patient advocate, providing support and information throughout the entire cancer pathway to reduce patient anxiety. You will provide reassurance and information on appointments, interventions/tests, ensuring patients arrive for interventions fully prepared. The post holder will work closely with the MDT coordinator to support and coordinate the care of cancer patients through their complex care pathways, ensuring all patients move through the pathway without unnecessary delays in order to achieve a definitive diagnosis by day 28.
The post holder will also track patients on cancer surveillance as part of the introduction of Patient Stratified Follow Up (PSFU).

The ideal candidate will be a confident, enthusiastic, dynamic and motivated individual with advanced IT, administrative, secretarial and organisational skills and cancer pathway knowledge. The post holder will be required to have excellent communication skills and an ability to deal with complex situations with service users and clinicians, both in primary and secondary care over the phone and in person.
