Are you a passionate, dynamic and compassionate practitioner ready to make a difference in young peoples lives? We are seeking a motivated and enthusiastic Band 5 mental health practitioner to join our CAMHS Crisis and Home Treatment Team in Hereford on a structured developmental pathway to Band 6. This is an exciting opportunity to work in a fast-paced, supportive environment that offers real scope for clinical development, training and supervision as you grow into a more senior role. As part of a multi-disciplinary team, you will provide responsive crisis assessments and intensive home treatment to children and young people experiencing mental health crises, with the aim of avoiding unnecessary hospital admissions. The team adopt a trauma informed holistic approach to the young people of Hereford and have a supportive, learning culture meaning this is a great place to work. The service runs 7 days a week 8am-8pm shifts, flexible working is encouraged. Any questions please reach out to Aly Irving.Main duties of the job Undertake crisis assessments and risk management plans for young people presenting in acute mental health distress Deliver time-limited, goal-focused interventions in community and home settings Collaborate with families, schools, social care, and acute services to provide holistic care Participate in team meetings, case discussions and multi-agency forums Maintain accurate clinical records and contribute to service development initiatives Participate in a 7-day service rota including evenings, weekends and bank holidays Participating in clinical, safeguarding and reflective supervision. Work alongside clinical leads and operational leads
