CAMHS Liaison Practitioner

We are looking for a practitioner who is confident working autonomously and accountably within an integrated team. You will provide comprehensive mental health assessment for children and young people who are in crisis, formulating details risk management plans and making referrals to partner agencies and services. You will thrive on working with a wide range of Children, Young People, Families, teams and services. You will be adaptable, able to meeting the changing needs of individuals, teams and networks.

You will provide a high level of specialist mental health advice and input within discharge planning meetings.

You will also be able to offer post-assessment reviews, psycho-social interventions, and self-harm minimisation tools, to effectively manage risk and support a young person's safety within the community.

The role involves shift work, over a 7-day period. Covering hours of 9am-10pm Monday to Friday and 9am-5pm weekends and bank holidays. Flexible working pattern available on request.

Main duties of the job

The successful candidate will undertake rapid crisis assessments for young people referred through the general hospital providing short crisis intervention and signpost to other specialist services as required. In this they will demonstrate a high level of skill in assessing and managing risk.They will be able to formulate and convey sensitive, complex, and sometimes unwelcome information to clients and other professionals and manage this as constructively as possible. They will be able to identify when a biopsychosocial assessment is required and when a risk-review or advice and guidance to parents and carers are appropriate. The successful candidate will be able to participate in and lead in joint complex assessments with other disciplines and other agencies as required.

The successful candidate will work collaboratively across the CAMHS directorate and partnership agencies. They will develop and maintain positive working relationships with all internal and external partners and stakeholders.They will identify and maximise on opportunities for consultation, specialist advice and training to professionals who come into contact and who have an important influence with children and young people who have mental health needs.

The successful candidate will have effective written communications skills to support the success of the clinical duties above. They will ensure documentation and care plans are comprehensive, timely, accessible and informative for the reader.
