Full Job Description

  • University Hospitals of North Midlands NHS Trust, inclusive of Black Country Procurement Group and North Midlands and Cheshire Pathology Service.
  • Staffordshire and Stoke-on-Trent Integrated Care Board
  • To offer needs-led support and signposting to families and children within the service. The care navigator will ensure that the children and families are well while waiting for their assessments by signposting parents to further support. To also liaise closely with the internal and wider multidisciplinary teams to ensure the family and young person have the correct support in place. To work together with other members of the MDT. This post is based at The Bridge, Stafford, but will include some potential for home visits to locations across South Staffordshire., To provide guidance to Family carers in order for them to access timely support and understanding of the assessment pathway process. Support the team to ensure that families are kept informed of the support and options available to them Main point of contact for families with a child on the waiting list and proactively aim to offer contact as required, individualised to the needs of the child and family/carers. To meet regularly with supervising clinician to review clinical issues, and with team leader for managerial support. Under supervision signpost unsuitable referrals to the relevant service as necessary. To identify issues to allow for timely individualised assessment, escalating to line manager where appropriate. To contribute to the assessment of and adhere to risk management measures under direction of senior clinicians. To access regular child protection supervision and work to trust safeguarding policy. To be a trusted link with partner agencies, the Family Hubs and the CAFSPA. To ensure information is available for children and parents/carers in a range of formats To participate in projects as required.
